is it best to ply with another spindle right off that spindle, or should the singles be removed and wound into a ball first? is either way fine–is it just personal preference?

It is easiest to do right off the spindle, it will twist easier.
Either way is fine.
Don’t try plying silk from a centre pull ball though, it will tangle upon itself.
